Dear Resident and Family members,

I would like to welcome you to Chelsea Place Care Center. At Chelsea Place, it is our mission to enrich the lives of the residents we serve. Our approach to resident care is of a holistic nature in that the "whole person" is treated as we strive to meet the physical, emotional, and psychosocial needs of all. At Chelsea Place, we will always strive to exceed your expectations. We hope to accomplish this by always leaving open lines of communication while the interdisciplinary team partners with you to develop your plan of care. This process is initiated with a "Meet and Greet" meeting that is scheduled just days after admission and continues with regularly scheduled care plan conferences and discharge planning meetings.

Our dedicated team at Chelsea Place is truly committed to providing for all of your needs and to enrich the lives of the residents and community we serve. Thank you for entrusting us to care for you and your loved ones.
